Prescription Refill and Food Order Request Form

Please use the form above to request your pet’s prescription refill or food item at our Seattle veterinary hospital. This will ensure that we have the item in stock and will save you time when picking up your order. Please allow 24 hours during business days for our customer service staff to contact you about your order. You may also place orders through the Hawthorne Hills Veterinary Hospital online store using your Pet Portal.

Going out of town? There is nothing more important than making sure your pet receives appropriate care in an emergency situation.

Please call or Email us for a form we can send for you to complete. This form will authorize your friend or pet care givers to make any urgent decisions on your behalf.

Hawthorne Hills Veterinary Hospital veterinarians will make every effort to contact you if your pet requires medical care at our facility while you are away from Seattle. If we are unable to contact you during your absence, it is important to be sure that your friend or pet care givers understand your wishes and are able to authorize treatment.
